I too struggle with this issue of a composite outcome that is ultimately driven by just one component. In less extreme examples, it may be worthwhile to test for heterogeneity of the effect across components to determine if this is likely to be true:

Iā€™m not not clear on why this concerns you. One alternative would have been too pick one component and to have your head completely in the sand about the other components.
